Object:  To evaluate risk factors and methods of infection control in surgical site infections in postoperative neurosurgical patients. Methods:  This study was prospective, uncontrolled and observational study conducted on patients with postoperative neurosurgical operation and admitted to Intensive Care Unit postoperatively.  The study was done in Al-Azhar university hospitals from March 2017 to March 2018. The study aim was. Analysis of data was done by IBM computer using SPSS (statistical program for social science version 25). Results:  In the study sample, the mean age and standard deviation (SD) were 42.5 years ±19.1. Female patients were about 40%. Most male patient were above median age of the sample (37.8 years). The overall admission period includes pre-ICU admission, ICU admission and post-ICU admission. The mean and standard deviation of overall admission was 43.7 day±5.5. From the whole study sample, nosocomial infection was estimated to be 88.4% of cases (69 patients). Postoperative mortality among cases was 36%. The rest of cases were either referred or discharged to ward. Comorbid conditions may present as single factor or multiple factors in the same patient. Patients with nosocomial infections were found to have two or more comorbid conditions while infection-free cases were found to have no or at least one factor in a statistically significant association. It has been found that most cases with traumatic brain injury and hemorrhagic stroke had nosocomial infection (74%). Both types mentioned previously had longer ICU stay.  It has been found that nosocomial infection is commonly predominant in traumatic brain injury and intracranial hemorrhage cases (ICH).  Nosocomial infections were found to be either single or multiple infections in the same person. Most of infections were; respiratory tract infection, urinary tract infection or septicemia. When the entire study sample was subjected to logistic regression analysis, age, diagnosis and hospital stay, necessity for ventilation, low Glasgow Coma Scale (GCS), high temperature, high humidity and low karnofsky were found to be significantly independently predictive of postoperative nosocomial infection. Among these patients, type and timing of operative intervention and environmental factors did not significantly affect outcome. Formosan Medical Association & Elsevier, 106(2), pp. 110–118. doi: 10.1016/S0929- إن العدوى المکتسبة في الرعاية المرکزة لمرضى جراحة المخ والاعصاب قد تؤدي الى زيادة فترة الرقود في الرعاية المرکزة وبالتالي زيادة ثقل اضافية على رد فعل الجسم لمقاومة مابعد العملية مما يوهن صحة المريض وقد يؤدي الى الوفاة. تعتبر عدوى الجهاز التنفسي, التناسلي والوعائي وکذلک عدوى الجروح الناشئة ما بعد العملية وقرح الفراش من اشکال العدوى البکتيرية محل الدراسة. ان هدف الدراسة الاساسيهو تقييم العوامل البيئية المؤدية الى حدوث العدوى المکتسبة في المرضى الذين ادخلوا غرف الرعاية المرکزة لجراحات المخ والاعصاب مابعد اجراء جراحة المخ او العمود الفقري وطرق مکافحة هذه العدوى بکافة اشکالها. شملت هذه الدراسة من هم فوق ال18 سنة وکلا الجنسين من الذين خضعوا لعمليات جراحة المخ والاعصاب والعمود الفقري فيما بعد العملية. تم جمع المعلومات المطلوبة واخضاعها للفحص والتحليل باستخدام برنامج الاحصائي اس بي اس اس النسخة 25. تم اخذ موافقة مجلس الاخلاقيات الطبية قبيل بدء الدراسة. تعتبر هذه الدراسة دراسة مسبقة, مسحية ولا تتضمن وجود مجموعة سيطرة فيها. اقيمت هذه الدراسة في مستشفيات جامعة عين شمس للفترة من مارس 2017-مارس 2018. وخلال مدة عام تم جمع 78 حالة. بلغ متوسط العمر الحسابي هو 42.5 سنة وانحراف معياري يبلغ 19.1. کان عدد الرجال اکثر من النساء حيث کان الرجال 47 فرداً. امضى افراد العينة فترة في الرعاية المرکزة بمتوسط 43.7 يوم بعد استبعاد عينتين قضتيا 3 اشهر. بلغت نسبة الالتهابات 88.4% من الحالات اي ما يعادل 69 حالة. نسبة الوفاة نتيجة الالتهاب بلغت 36%. لقد وجد احصائياً ان الحالات المصابة بالالتهاب (اياً کان نوعه) قد مکثت بشکل اطول في الرعاية المرکزة عن تلک الحالات التي لم تصب.  عندما تم اخضاع المتغيرات ذات العلاقة الترابطية مع حالة الالتهاب لدراسة الانحدار بين المتغيرات والنتيجة وجد ان العمر والتشخيص والمکوث في المستشفى قبيل العملية والحاجة للتنفس الصناعي وانخفاض درجة الوعي  وارتفاع الرطوبة ودرجة حرارة الجو داخل غرفة الرعاية المرکزة مع انخفاض مؤشر کارنوفسکي کانت اسباب مهمة لحدوث الالتهاب داخل الرعاية المرکزة. ان اسباب حدوث الالتهاب داخل الرعاية هي عديدة بالنسبة لمريض تم علاجه بواسطة تداخل جراحي عصبي, هؤلاء المرضى يحتاجون الى التقييم ماقبل العملية ومراجعة کافة الاطر العلمية التي على اساسها يتم معالجة المريض في الرعاية المرکزة والمتعلقة بدرجة الحرارة والرطوبة وعدد الاسرة وعدد الممرضات اللازم لکل سرير وخلافه لتقليل نسبة الاصابة بالعدوى.}, keywords_ar = {}, url = {https://jes.journals.ekb.eg/article_166257.html}, eprint = {https://jes.journals.ekb.eg/article_166257_4c0a23d126be602b5bcefe1937070d51.pdf} } @article { author = {Zahran, Wafaa and Awad Allah, Hala and El Guindy, Alaa El Din}, title = {ASSOCIATION OF METABOLIC SYNDROME WITH DEVELOPMENT OF PRE-ECLAMPSIA AMONG PREGNANT WOMEN}, journal = {Journal of Environmental Science}, volume = {49}, number = {12}, pages = {30-55}, year = {2020}, publisher = {Ain Shams University, Faculty of Graduate Studies and Environmental Research}, issn = {1110-0826}, eissn = {2636-3178}, doi = {10.21608/jes.2020.166258}, abstract = {Background: Preeclampsia is a complicated disorder in pregnancy which occurs after 20 weeks of gestation. It affects 3-5% of all pregnancies in the world. It has been reported that the prevalence of preeclampsia has been altered between 1.8% and 16.7% in developing countries Women with preeclampsia seem to be at elevated risk for cardiovascular disease. Objective: This study was doneto determine the relation between metabolic syndrome and occurance of pre-eclampsia and to evaluate the use of metabolic score in determining the devolpment and severity of pre-eclampsia. Methods: This is a case-control study includes women attending to obstetric emergency unit in labour.  The period of study was about 24 month from January 2017 to December 2018. The cases were divided into two groups: The first group consists of 300 cases with pre-eclampsia which characterized by: Mild pre-eclampsia as blood pressure higher than 140/85 and protein 1+ or greater by drip stik, moderate pre-eclsampsia blood pressure is more than 140/85 and less than 160/100 and protein 1+ or greater by drip stik. Severe pre-eclampsia (imminant-eclampsia) or blood pressure is higher than 160/100 and protein greater than 3+ by drip stik. The second group consists of 300 control patients with normal pregnancy. Results: Preeclampsia was a dependent risk factors for metabolic syndrome (adjusted odds ratio = 6.888, 95% CI = 4.656 to 10.189, P-value <0.0001). While age category 40-49 years was associated with significantly lower risk for metabolic syndrome as referenced to age category 18-29 years adjusted odds ratio = 0.337, 95% CI = 0.168 to 0.677, P-value = 0.002). Childhood malnutrition is a major concern in developing countries. Aim of the study: This study aimed to determine the change in the knowledge of children’s mothers at school age in ElothmanyaVillage- El Gharbyia Governorate after an interventional program. Patients and methods: This study targeted two groups: The first group included obese children aged 8 to 15 years old and the second group included underweight children aged 7 to 16 years old using questionnaires, educational tools and model kitchen. Results: In obesity group, mothers’ age ranged from 23 to 46 years with mean of 32.3 ± 5.7. Most of them were housewives. Most of fathers were workers and farmers in both groups. Attendance of mothers of obese children was as follows; 41.3% for once, 28.3% for twice, 17.4% for 6 times. Mean age of obese children was 10.8 ± 2.4. Mean BMI was 27.8 ± 1.7 and ranged from 26 to 34. Knowledge of mothers generally improved with average of 33.3% regarding obesity. Knowledge regarding obesity complications was found to be the most improved item when comparing it before and after intervention. In underweight group, Mothers’ age ranged from 24 to 52 years with mean of 36.7 ± 6.1. Attendance of mothers of underweight children was as follows; 29.5% for once, 32.1% for twice, 14.1% for 3 times, 11.5% for 4 times, 6.4% for both 5 and 6 times. Mean age of Underweight children was 11 ± 1.9. Mean WAZ score  was -1.8 ±0 .8 and ranged from -3.09 to -1.02. Knowledge of mothers generally improved with average of 37.3% regarding underweight. Knowledge regarding Number of meals was found to be the most improved item when comparing it before and after intervention.
